Full-time Physical Therapist (PT) to manage our rehab services. This position will report directly to the Director of Professional Services and serve as the rehab liaison with referring physicians and nurse case managers. This position will make patient admissions, visits, and discharges, and supervise LPTA’s and aides as required. This position will work in conjunction with our case managers to meet the goals of current plans of treatment, and will provide inservices as needed for our clinical staff.

Full-time Licensed Physical Therapy Assistant (LPTA). This position will report directly to the Managing Physical Therapist making patient visits as requested.

Full-time staff RN Case Manager. This position will report to the Director of Professional Services and the Clinical Coordinator. This position will be responsible for admitting, creating a plan of care/treatment for patients, managing the care needed by patients and working with staff LPN’s and aides to provide patient care. This position will be responsible for all patient care documentation, supervisory visits, recertifications, post-hospital evaluations, transfers, and discharges. Home health experience is preferred but not required. Nursing home and medical surgical experience is required.

Contract/Visit/PRN Staff RN. This position will provide backup for full-time RN staff, and assist in coverage of patients when full-time staff are not available. This position will be responsible for making patient visits and documenting admissions, post-hospital evaluations, recertifications, transfers, and discharges.

Home Health Aide (HHA). This position requires a 120-hour Nursing Assistant Certificate, or a Virginia license as a Certified Nurse Aide (C.N.A.). This is a visit paid position requiring visits to several patients a day. This position will require extensive travel, we reimburse for mileage.

Nursing Assistants. This position requires an aide certificate (Nurse Aide, Nursing Assistant, Geriatric Nursing Assistant, or Certified Nurse Aide) or a license as a Certified Nurse Aide (C.N.A.). These positions are available for the following areas: Scottsville, Wingina, Nelson County areas, and Cumberland. Must be willing to participate in weekend duty.

Personal Care Aides. This position requires a 40-hour aide certificate. We have Personal Care patients in all areas of our service area: southern Albemarle County/Scottsville, Wingina, Fluvanna, Buckingham, Cumberland, and Nelson County areas. Must be willing to participate in weekend duty.

Personal Care Aide Class. Every so often we offer the Personal Care Aide Class. It is a 40-hour course offered solely for recruitment purposes. It is offered when our nurses are available to teach the class. Please call or come by our office for a Personal Care Aide Class Application form. Students eligible for the class must have a current active Virginia driver’s license, have dependable transportation, and able to demonstrate some type of work experience with valid work references. Students for the class must be willing to work for our agency upon completion of the class, a minimum of 30 hours per week. Must be willing to participate in weekend duty.

We will not teach anyone who has a felony criminal record. All students of the class are required to wear scrubs, lace-up shoes, no flip-flops or sandals. No long finger nails, only minimal jewelry will be allowed!
